Health & Wellness Waiver Services in Indiana

The Health & Wellness Waiver (H&W) provides home and community-based services for individuals with physical disabilities and older adults who need long-term support to remain safely in the community. CTI is a certified H&W Waiver provider offering Structured Family Caregiving, Participant Assistance & Care, and other HCBS services under this waiver.

Who qualifies: Individuals with physical disabilities or complex health needs | Age 18+ in most cases | Indiana Medicaid eligible | Requires medical and functional assessment by FSSA
